This notice describes how medical information about you may be used and disclosed and how you can access this information. Please review it carefully.

Your protected health information, including individually identifiable information such as names, dates, phone numbers, fax numbers, email addresses, home addresses, Social Security numbers, demographic data, treatment information, and billing information, may be used or disclosed by us in one or more of the following ways:

  1. To other health care providers, such as your general dentist, oral surgeon, physician, or other treating provider, in connection with our rendering orthodontic treatment to you.
  2. To third-party payors or responsible parties, including insurance companies, employers with direct reimbursement arrangements, administrators of flexible spending accounts, spouses, parents, or guardians, in order to obtain payment for your account, determine benefits, confirm eligibility, or process claims.
  3. To certifying, licensing, and accrediting bodies, including the American Board of Orthodontics, state dental boards, or similar organizations, in connection with certification, licensure, accreditation, compliance, or professional requirements.
  4. Internally, to staff members who have a role in your treatment, scheduling, billing, insurance, administration, or patient care.
  5. To other patients and third parties who may see or overhear incidental disclosures related to your treatment, scheduling, or office communications, although we make reasonable efforts to limit such disclosures.
  6. To your family members, close friends, parents, guardians, or other individuals involved in your treatment, payment, or care, where appropriate and permitted by law.
  7. To contact you with appointment reminders, treatment updates, scheduling information, billing information, insurance information, or information about treatment alternatives, services, or health-related benefits that may be of interest to you.

SMS, Mobile Phone Numbers, and Text Messaging

If you provide your mobile phone number to this office, we may use it to contact you by phone call or text message for purposes related to your care, including appointment reminders, scheduling updates, treatment-related messages, billing notifications, or other practice-related communications.

We do not sell, rent, share, or disclose mobile phone numbers or SMS consent information to third parties or affiliates for marketing or promotional purposes.

Mobile phone numbers and SMS consent information are used only for communications related to this office and your care, unless otherwise required or permitted by law. Message and data rates may apply depending on your mobile carrier and phone plan.

You may opt out of receiving text messages at any time by replying STOP to any text message you receive from us, or by contacting our office directly. Please note that opting out of text messages may limit our ability to send you convenient appointment reminders or other time-sensitive communications by text.

You have the right to revoke your written authorization at any time, except to the extent that we have already relied upon it.
